3. Fill out and send to the address on the bottom of the form along with your deposit.

2. Fill out electronically, and save changes.

3. Email it back to us a cameronplacecsa@bresnan.net as an attachment

4. Drop a check in the mail for required amount.

Please note that to secure the registration, the form must be recieved by January 31st, 2010.
